Your first visit is the foundation of our future relationship. During your first visit, we usually go over your history and get to know you. There are a few steps you can take to make the process easier and quicker. Please,
1. Download the following registration forms, print, and fill them out, and bring them with you to your first visit.

2. Take a picture of the back and front of your insurance card and send it to us at howardjlernerdmd@gmail.com.
3. Contact your previous dentist and ask them to forward us your dental records including your recent x-rays at howardjlernerdmd@gmail.com.

What to Expect
On your first visit, usually a full set of x-rays are taken. Dr. Lerner will go over your medical and dental history. After reviewing your x-rays and a comprehensive dental and oral examination, he will provide his professional feedback along with all possible options, so he and you could move forward by selecting the best treatment for you. Dental cleaning can usually be performed on the same visit.
